There is a quest NPC named Shadowalker Dustspirit in Stoneroot Falls who gives a series of quests, the first quest is "Freeing an Elder", and the second is called "The Orb of Subversion." In both missions, he gives you an instanced version of the Hive and I believe that the mask drops in either of these missions as a rare zonewide drop.

A friend of mine got one in the hard version of Freeing an Elder, and he is the only one I know who has seen the mask drop. Sorry, I don't know if it can drop in the normal version :confused:
