Getting a title before/after your name.
If you have obtained a title either slaughtering, questing or /claiming it will be avalible for you in your persona. Just open up your persona (P) and then pick your title from the drop down lists.
Surnames
When you get level 20 to obtain a surname just type /lastname (name) to recieve it.
Gods Name Suffixes
These are from people who've played Everquest 1. They get an inheritance of sorts. One of the things they get to claim is a Server/God Title and the other is a painting by typing /claim.
GM given titles
Protector, Conqueror... FYI - The Conqueror title (as well as the Protector title) was given out by GM's during the live events leading up to the release of "The Bloodline Cronicles".
Players In Guilds With Higher Levels Get Prefixes
As a guild lvls up certain perks become avalible to them. At lvl 30 the first set of Nobility Titles become avalible.
Male: Freeport: Risar, Qeynos: Sir
Female: Freeport: Dame, Qeynos: Madam
Hunters, Slayers & Destroyers
In order to get these titles you need to kill a certain number of mobs. There are many different titles for cetain mobs.

In order to see the number of kills you have you can head to the wanted poster in your local town.
Qeynos poster can be found in North Qeynos just inside the Antonica gates on the post to the right as you enter the city.
Freeport poster can be found in West Freeport, on the north-east corner of the execution plaza (across the gate to North Freeport).

250 - Accomplished Hunter

By completing a certain number of heritage quests, you can obtain more titles. You need to talk to the city Sage.
Qeynos - Sage Indis Surion in the Mage Tower in South Qeynos (yellow sparkly lights)
Freeport - Sage Naravius Orvalis in the Academy of Arcane Science in North Freeport
Treasure Hunter – 5 heritage quests
Lore Seeker - 10 heritage quests
Relic Keeper - 20 heritage quests
The "of the Shard" title
That title is claimable after completion of the Deception quest from Lord Nagafen in Solusek's Eye.
